HealthMeek’s Terms are written to make expectations clear before care begins. They protect clients, families, caregivers, staff, and the company by explaining what HealthMeek does, what it does not do, and what users must do responsibly.

Request Care

Clear public terms.

These terms explain website, portal, care request, caregiver, billing, and service expectations.

Not emergency care.

HealthMeek is not an emergency service. Urgent danger requires 911 or local emergency services.

Protection for everyone.

The terms support safer care, respectful communication, payment clarity, and secure portal use.

Acceptance of These Terms

By using the HealthMeek website, submitting a form, requesting care, accessing a portal, applying as a caregiver, or using HealthMeek services, you agree to these Terms of Service.

These Terms apply to website visitors, clients, family members, caregivers, applicants, staff portal users, and anyone using HealthMeek online services.

If you do not agree with these Terms, do not use the website, portals, forms, or services.

Separate service agreements, employment documents, care authorizations, billing terms, consent forms, or addenda may also apply.

If another signed agreement conflicts with this public page, the signed agreement may control for the specific matter it covers.

HealthMeek may update these Terms as services, portals, workflows, laws, or business needs change.

HealthMeek Services

HealthMeek provides home care support, care coordination, family communication, caregiver workflows, and related office services.

Services may include personal care, companion care, respite care, memory support, transportation support, pet care support, and structured care programs.

Service availability depends on location, staffing, scheduling, client needs, safety considerations, and office approval.

Submitting a request does not guarantee acceptance, start of care, a specific caregiver, exact schedule, or a particular service outcome.

HealthMeek may decline, pause, modify, or discontinue services if safety, staffing, payment, compliance, scope, or care needs cannot be appropriately managed.

Care begins only after HealthMeek reviews the request and completes the required intake, authorization, agreement, and scheduling steps.

Not Emergency or Hospital Care

HealthMeek is not an emergency service, hospital, ambulance provider, or substitute for urgent medical care.

For emergencies, call 911 or local emergency services immediately.

Do not use website forms, portal messages, email, voicemail, or text messages for emergencies.

HealthMeek does not guarantee real-time monitoring or immediate response to website or portal messages.

HealthMeek services are not a replacement for physician orders, emergency medical treatment, hospital care, or licensed medical advice.

If a client has chest pain, trouble breathing, severe injury, sudden confusion, signs of stroke, severe bleeding, or other urgent symptoms, call emergency services immediately.

Eligibility, Intake & Service Approval

HealthMeek may require intake review, documentation, authorization, and office approval before services begin.

Clients or authorized representatives must provide accurate and complete information during intake and service updates.

HealthMeek may request emergency contacts, care preferences, access instructions, pet information, safety notes, mobility details, and relevant support information.

The office may review whether requested services are within HealthMeek’s scope and whether appropriate staffing is available.

HealthMeek may require signed agreements, consents, policies, payment setup, or other documents before services begin.

Incomplete, inaccurate, unsafe, or unauthorized information may delay or prevent service approval.

Client & Family Responsibilities

Clients, families, and authorized representatives must help keep the care environment safe, accurate, respectful, and workable.

Provide truthful information about needs, risks, routines, behavior concerns, access instructions, pets, and household conditions.

Maintain a safe environment for caregivers and HealthMeek staff.

Treat caregivers and office staff respectfully and avoid harassment, discrimination, threats, unsafe conduct, or inappropriate requests.

Notify HealthMeek promptly about schedule changes, hospitalizations, falls, safety concerns, medication changes, contact changes, or major care changes.

Ensure an authorized person is available when required for access, decisions, payment, or care coordination.

HealthMeek may pause or discontinue services if the environment becomes unsafe, abusive, unauthorized, or outside service scope.

Caregiver & Applicant Responsibilities

Caregivers, applicants, and team members must provide accurate information, follow HealthMeek policies, and maintain professional conduct.

Applicants must provide truthful qualifications, identity information, credentials, availability, and onboarding documents.

Caregivers must follow assignment instructions, confidentiality requirements, safety procedures, documentation expectations, and office communication policies.

Caregivers may not accept unauthorized private side arrangements that conflict with HealthMeek policies or client agreements.

Caregivers must report incidents, schedule issues, care concerns, unsafe conditions, and required updates according to HealthMeek procedures.

Caregiver access to assignments, dashboards, clients, documents, and portals may be limited, suspended, or removed if policies are violated.

Portal Accounts & Login Security

HealthMeek portals are provided for authorized users only. Users are responsible for protecting their login credentials.

Portal users must use their own account and may not share passwords or access with unauthorized people.

Users must notify HealthMeek if they believe login credentials, portal access, or private information has been compromised.

HealthMeek may restrict, suspend, or terminate portal access to protect privacy, security, clients, caregivers, staff, or company operations.

Portal information may not always be complete, final, or updated in real time.

Portal access is a convenience and communication tool. It does not replace direct office communication for urgent or time-sensitive matters.

Scheduling, Cancellations & Changes

Schedules depend on staffing, caregiver availability, client needs, safety, traffic, weather, emergencies, and office approval.

HealthMeek will make reasonable efforts to coordinate approved services, but exact schedules and caregivers are not guaranteed.

Clients or representatives must request cancellations, changes, or additional services according to HealthMeek policy or signed agreement.

Late cancellations, missed visits, denied access, or schedule changes may result in fees depending on the service agreement.

HealthMeek may adjust visits because of caregiver availability, illness, emergency, safety concerns, weather, transportation issues, or operational needs.

Care schedules should be confirmed through the HealthMeek office or approved portal workflow.

Payments, Billing & Fees

Clients or responsible parties agree to pay approved service charges, deposits, invoices, fees, and balances according to HealthMeek billing terms.

Pricing may vary by service type, schedule, duration, location, urgency, holidays, special needs, transportation, supplies, or program structure.

Payment may be required before services begin or according to an approved billing arrangement.

Late payments, unpaid balances, declined payments, or billing disputes may delay, pause, or terminate services.

Refunds, deposits, cancellation fees, minimum visit charges, and billing adjustments may be governed by separate service agreements or billing policies.

Clients are responsible for reviewing invoices and raising billing concerns promptly.

Do not send full payment card numbers, bank passwords, or sensitive payment credentials through regular email, text, or public website forms.

Transportation & Errand Support

Transportation, errands, and outside-the-home support may require additional rules, authorization, safety review, and scheduling approval.

Transportation support is not emergency ambulance service.

HealthMeek may require details about mobility, destination, escort needs, appointment timing, wait time, access, and return arrangements.

Service may be declined or modified if the transportation or errand request is unsafe, unauthorized, unclear, outside scope, or not properly scheduled.

Clients and representatives are responsible for providing accurate destination, contact, access, and appointment information.

Transportation support should be arranged in advance whenever possible and confirmed through approved HealthMeek channels.

Pet Care Support

Pet care support is limited, non-veterinary assistance intended to help clients maintain comfort and routine at home.

Pet care support may include light feeding reminders, basic routines, or limited walking support when approved.

HealthMeek does not provide veterinary care, aggressive animal handling, grooming, boarding, medication administration for pets, or unsafe pet services unless expressly approved.

Clients and families must disclose pet type, behavior concerns, bite history, access needs, and household safety details.

HealthMeek may decline pet support if animal behavior, allergies, home conditions, or safety risks cannot be reasonably managed.

Pet care support may require a separate addendum or service approval.

Communication Consent

By providing contact information, users authorize HealthMeek to communicate by phone, text, email, voicemail, portal notice, or similar tools.

Communications may include care request follow-up, portal access, appointments, caregiver onboarding, assignments, billing, service updates, reminders, and office notices.

Message and data rates may apply depending on the user’s carrier.

Users may request to stop non-essential promotional messages.

Some service-related communications may still be required to manage care, safety, account access, billing, legal records, or operations.

HealthMeek cannot guarantee that regular email or text messages are fully secure, so users should avoid sending unnecessary sensitive information through those channels.

Website & Portal Content

Website, portal, and educational content are provided for general information and service coordination.

Public website content is not medical advice, legal advice, financial advice, or a guarantee of service availability.

HealthMeek may update, remove, correct, or change website and portal content at any time.

Errors, outdated information, temporary outages, or display issues may occur.

Users may not copy, misuse, scrape, attack, reverse engineer, interfere with, or abuse HealthMeek systems or content.

Use of HealthMeek online systems must remain lawful, respectful, secure, and consistent with the intended purpose of the service.

Privacy & Confidentiality

Use of HealthMeek services is also governed by HealthMeek privacy practices and confidentiality expectations.

Users must respect the privacy of clients, caregivers, family members, staff, and portal users.

Caregiver and staff users may not disclose client information except as permitted by HealthMeek policy and applicable law.

Family and portal users must not misuse, share, or publish private care information without authorization.

HealthMeek may monitor, log, or review system activity to protect privacy, safety, and security.

Limitations & No Guarantees

HealthMeek works to provide compassionate, organized care support, but does not guarantee specific outcomes.

HealthMeek does not guarantee a specific caregiver, schedule, response time, health result, independence outcome, safety result, or family outcome.

Services depend on accurate information, cooperation, staffing availability, service scope, client condition, household safety, and outside factors.

Technology systems, portals, payment systems, emails, text messages, and websites may experience outages, delays, or errors.

HealthMeek is not responsible for events outside its reasonable control, including emergencies, weather, third-party failures, power outages, internet failures, traffic, or government action.

Nothing on the website should be interpreted as a promise that any specific care result will occur.

Disputes, Concerns & Service Issues

HealthMeek encourages clients, families, caregivers, and users to report concerns quickly so the office can review and respond.

Service concerns should be reported to the HealthMeek office as soon as possible.

Billing concerns should be raised promptly after an invoice or charge is received.

Caregiver assignment concerns, safety issues, complaints, incidents, or suspected abuse must be reported immediately through approved channels.

HealthMeek may investigate complaints, review records, contact involved parties, and take appropriate operational action.

For suspected abuse, neglect, exploitation, or emergency danger, contact the appropriate authorities or emergency services immediately.

Changes, Suspension & Termination

HealthMeek may update services, policies, portal access, pricing, pages, workflows, or Terms when needed.

HealthMeek may suspend or terminate website, portal, caregiver, client, or service access for policy violations, unsafe behavior, nonpayment, fraud, misuse, abuse, security concerns, or legal reasons.

Users may stop using the website or portal at any time, but signed agreements, payment obligations, confidentiality duties, and recordkeeping rules may continue.

HealthMeek may revise these Terms by posting updated Terms on the website.

Continued use of the website, portals, forms, or services after updates means the updated Terms apply.

Some obligations survive termination, including payment duties, confidentiality, privacy, record retention, dispute terms, and limitations of liability where permitted.
